A seven-days course of cotrimoxazole is the reference treatment; alternatives are ciprofloxacin and pyrimethamine. In case of malabsorption, intra-venous treatment should be given. A prophylaxis using the same drugs, half-dose, is necessary to prevent recurrent isosporiasis, whatever the CD4 level and for undetermined duration.
