Retrospective review of laser therapy for palliation of colorectal tumours

SummaryPatients with inoperable colorectal tumours will often require symptomatic relief due to the nature of extensive disease spread or existing co-morbidities. The use of laser treatment for palliation of tumours in the lower gastrointestinal tract has become an attractive treatment option for such patients. This paper presents the results of a retrospective review of 58 case notes in order to determine the effectiveness of laser therapy in palliating symptoms of colorectal tumours. In addition, the paper aims to identify which colorectal symptoms laser is best used to palliate. The study was conducted in a regional coloproctology unit at the Western General Hospital in Edinburgh. The main findings show that 52% (n=30n=30) of patients had successful (complete/good) resolution of symptoms, 36% (n=21n=21) had a poor response and 12% (n=7n=7) had no resolution of symptoms from laser therapy. Of all documented symptoms, this study found that laser is most effective at palliating obstructive symptoms. It also has beneficial application in the palliation of bleeding and mucous discharge. It is less effective for the anal symptoms of tenesmus and pain and for stool related symptoms such as diarrhoea, constipation, frequency and incontinence.

ZusammenfassungPatienten mit inoperablen kolorektalen Malignomen benötigen wegen des Fortschreitens ihres Tumorleidens oder aufgrund von Begleiterkrankungen häufig eine symptomatische Behandlung. Bei diesen Patienten hat die palliative Lasertherapie zu einer attraktiven Methode zur Behandlung von Tumoren des unteren gastrointestinalen Trakts entwickelt. Mit den in diesem Artikel vorgestelten Ergebnissen einer retrospektiven Review von 58 Kasuistiken wird das Ziel verfolgt, die palliative Wirkung einer Lasertherapie auf die Symptome kolorektaler Tumore zu evaluieren. Darüber hinaus haben die Autoren versucht, diejenigen Symptome kolorektaler Tumore zu identifizieren, welche am besten auf eine palliative Lasertherapie ansprechen. Die Studie wurde in einer regionalen koloproktologischen Abteilung am Western General Hospital in Edinburg durchgeführt. Die Ergebnisse zeigen, dass bei 52% (n=30) der Patienten eine erfolgreiche (d.h. vollständige/gute) Symptomlinderung erzielt wurde; 36% (n=21) der Patienten zeigten eine schlechte Linderung, und bei 12% (n=7) führte die Lasertherapie zu keinerlei Symptomlinderung. Außerdem wurde festgestellt, dass unter allen aufgetretenen Symptomen obstruktive Zeichen am besten auf eine palliative Lasertherapie ansprachen. Diese hat ferner einen günstigen palliativen Effekt auf Blutungen und Schleimexkretionen. Weniger wirksam ist die palliative Lasertherapie bei Symptomen wie Tenesmen und Schmerzen im Analbereich sowie bei Durchfällen, Obstipation, Stuhldrang und Stuhlinkontinenz.
