Neurophysiologische Diagnostik des Beckenbodens Methodik und Anwendungen

ZusammenfassungDie neurophysiologische Diagnostik des Beckenbodens trägt wesentlich zur Erfassung neurologischer Störungen von Blasenfunktion, Sexualfunktionen und Mastdarmfunktion bei. Am wertvollsten ist die Elektromyographie des Musculus sphincter ani externus, die nicht nur Läsionen des N. pudendus, des Plexus sacralis oder der Cauda equina nachweisen kann, sondern auch Störungen der zentralnervösen Innervation des Sphincters darstellen kann. Weitere Methoden sind der Bulbokavernosusreflex, die distale motorische Pudenduslatenz, somatosensibel evozierte Potentiale des N. pudendus und motorisch evozierte Potentiale zur Beckenbodenmuskulatur. Beschrieben wird einerseits die Methodik dieser Untersuchungen, andererseits auch deren Einsatz bei verschiedenen klinischen Fragestellungen unter Berücksichtigung der Pathopyhsiologie neurologischer Funktionsstörungen von Blase, Mastdarm und Sexualorganen.

SummaryElectrophysiological diagnosis of the pelvic floor ist important in the assessment of neurological causes of bladder dysfunction, sexual dysfunction and disorders of defaecation. Most valuable is electromyography of the external anal sphincter. Electromyography of the anal sphincter allows for the assessment of lesions of the pudendal nerve, the sacral plexus, the cauda equina and even central nervous sphincter control. Other methods are the pudendal distal motor latency, the bulbocavernosus reflex, somatosensory evoked potentials of the pudendal nerve and motor evoked potentials of the pelvic floor muscles. This paper not only describes the various neurophysiological methods but also shows how these methods can be used to solve diagnostic problems in the clinical context. In this respect a summery is given about the neurological pathophsyiology of bladder, sexual organs and rectum.
