Troubles du rythme cardiaque : diagnostic et prise en charge

RésuméLes troubles du rythme cardiaque, avec comme chef de file la fibrillation atriale, sont fréquents et peuvent être associés à une morbi-mortalité cardiovasculaire significative. Chaque praticien sera amené à intervenir à différentes étapes de la prise en charge (du diagnostic au traitement), sans toujours faire appel au spécialiste. Cette mise au point, synthétique, a pour objectif de présenter la physiopathologie des principaux troubles du rythme, leurs présentations cliniques et électrocardiographiques, ainsi que la conduite à tenir diagnostique et thérapeutique. Seront abordées dans l’ordre les tachycardies supra-ventriculaires (fibrillation et flutter atriaux, tachycardies jonctionnelles) puis les tachycardies ventriculaires.

Cardiac arrhythmias, with, on top of the list, atrial fibrillation, are frequent conditions and any physician might have to get involved at any stage of patient care (from diagnosis to treatment), without always having the opportunity to immediately refer to the cardiologist. The aim of this review is to present a summary of pathophysiology, clinical and electrocardiographic presentations, as well as diagnostic and therapeutic strategies for the main cardiac arrhythmias. Supra-ventricular tachycardias (atrial fibrillation and flutter, atrioventricular reciprocating tachycardias) and ventricular tachycardias will be consecutively presented and discussed.
