Evolución de los pacientes con cáncer de mama que sobreexpresan el receptor del factor de crecimiento epidérmico humano (HER2/Neu) en el Hospital del Instituto de Seguridad y Servicio Social de los Trabajadores al Servicio de los Poderes del Estado de Pue

ResumenPropósitoEl objetivo de esta revisión es saber la evolución de las pacientes que fueron tratadas en el hospital ISSSTEP, con cáncer de mama, la mayoría de ellas recibieron trastuzumab, ya sea en adyuvancia o para el tratamiento de la enfermedad metastásica.Material y métodosTomamos en cuenta a las pacientes que sobreexpresaron el gen HER2/Neu, ya sea por inmunohistoquímica o por FISH, teniendo como criterios la edad, etapa clínica, y la evolución de su enfermedad. Esta revisión abarca del año 2009 a finales del año 2014.ResultadosLas pacientes con cáncer de mama reportadas fueron 238, de las cuales recabamos 187 con HER2/Neu negativo, y 51 pacientes con HER2/Neu positivo. Las pacientes que presentaron metástasis, ya sea al inicio de su enfermedad, o como parte de su recurrencia fueron 37 en total, siendo 26 para el grupo HER2/Neu negativo y 11 para el grupo HER2/Neu positivo.El sitio de metástasis de las pacientes HER2/Neu positivas, fueron: 5 en sistema nervioso central, 5 a piel, 4 a pulmón, 3 a hueso y 2 a hígado.ConclusiónNuestro grupo nos indica que el sitio más frecuente de metástasis es el sistema nervioso central y piel. Basado en esta experiencia consideramos que los pacientes que sobreexpresan HER2/Neu deben tener una tomografía basal cerebral y debe vigilarse estrechamente su evolución, así como de la sintomatología neurológica, o realizando tomografías periódicas para detectar metástasis tempranas.

Most of them received trastuzumab, either adjuvant or for the treatment of metastatic disease.MethodsWe take into account the patients expressed about HER2/Neu gene, either by immunohistochemistry or FISH, with the criteria of age, clinical stage, and the evolution of his illness. This review covers the year 2009 by the end of 2014.ResultsThe patients with breast cancer were reported 238, of which 187 collect HER2/Neu negative, and 51 patients with HER2/Neu positive. Patients who developed metastases, either at the beginning of his illness, or as part of their recurrence were 37 in total, with 26 for the HER2/Neu negative group and 11 for the HER2/Neu positive group. The metastatic sites of HER2/Neu positive patients were: 5 in central nervous system, 5 to skin, 4 to lung, 3 to bone, and 2 to liver.ConclusionOur group, indicates that the most common site of metastasis is the central nervous system and skin. Based on this experience we believe that patients who over-express HER2/Neu should have baseline brain scans, and their developments as well as neurological symptoms should be closely monitored, or perform periodic scans to detect early metastasis.
