Agranulocytose et vascularite chez un patient cocaïnomane : lévamisole ou l'autre poison caché

RésuméIntroductionLe lévamisole est une substance adultérante, produit utilisé pour « couper » la cocaïne notamment, de plus en plus répandu en France comme aux États-Unis. Antihelminthique vétérinaire aux propriétés immunomodulatrice, il est aussi responsable de neutropénie sévère et de vascularite nécrosante, par atteinte des petits et moyens vaisseaux.ObservationNous rapportons ici le premier cas français de toxicité attribuée au lévamisole, liée à la consommation de cocaïne chez un homme de 50 ans, révélée par une fièvre avec agranulocytose, avec ANCA, anticoagulant circulant de type lupique et test de Coombs positifs. L'évolution était lentement favorable à l'arrêt de l'intoxication.ConclusionLa survenue d'une agranulocytose ou d'une vascularite cutanée chez un consommateur régulier de cocaïne doit faire évoquer une intoxication au lévamisole.

IntroductionAdulterants are compounds added to street drugs to increase profits for the seller. Levamisole, a veterinary antihelminthic agent, has become the most common adulterant of cocaine. The prevalence of levamisole in samples of cocaine is increasing. Levamisole can lead to neutropenia and to a dramatic vasculopathy and even vasculitis of small and medium-size blood vessels.Case reportWe here reported the first French case of levamisole related toxicity, due to cocaine use in a 50-year-old man, revealed by fever and agranulocytosis, high titters of antineutrophil cytoplasmic antibodies (ANCA), anticoagulant and positive Coombs tests. Outcome was slowly favorable with exposition withdrawal.ConclusionClinicians should be aware that agranulocytosis or vasculitis or vasculopathy could be related to levamisole toxicity in individuals who use cocaine.
