Les permanences d'accès aux soins de santé (PASS) : tradition d'hospitalité et modèle d'organisation innovante

RésuméLes permanences d'accès aux soins de santé (PASS) sont des dispositifs de consultations hospitalières facilitant l'accès aux soins aux patients démunis. Reposant sur la polyvalence et le décloisonnement médico-social, les PASS proposent les réponses les mieux adaptées aux situations complexes et polymorphes et participent à la réduction des inégalités de santé. Les PASS illustrent la place complémentaire essentielle d'une approche transdisciplinaire dans une médecine hospitalière marquée par une segmentation en spécialités d'organes. Véritables interfaces ville-hôpital opérationnelles, les PASS devraient devenir des acteurs clés de l'optimisation des parcours de santé et le modèle d'une médecine de parcours. Elles ambitionnent de devenir un modèle de médecine ambulatoire hospitalière permettant de rationaliser le recours à l'outil hospitalier, en particulier aux urgences et aux services d'hospitalisation conventionnelle. Tenant compte de leurs potentialités, les PASS constituent un véritable laboratoire d'innovation ouverte (living lab) de recherche, d'expérimentation et de prototypage d'un modèle innovant d'organisation des soins.

The PASS (“Permanence d'Accès aux Soins de Santé”) are hospital-based units providing primary care services to patients who lack health care coverage. Using a “whole person” approach and providing a combination of health and social care, the PASS offer an appropriately adapted response to complex health problems within a context of marked social vulnerability and contribute to reducing health inequalities. The PASS are an example of an interdisciplinary approach to health care which contrasts with the segmentary approach typical of conventional hospital departments. Operating at the interface between primary and secondary care, the PASS have the potential to become key players in developing models of patient pathways. Their presence reduces inappropriate emergency attendances and hospitalisation by offering medical care in a timely fashion, in an outpatient-type setting. The PASS can provide a resource for research into optimum models of health care, where the social context of health needs are fully recognized and inform medical treatment appropriately. According to their potential development, PASS are living labs of an innovative organizational model of care.
