Néphropathie aux anti-vitamine KÂ : Ã propos d'une observation

RésuméIntroductionLa néphropathie liée à la warfarine (WRN), de description récente, se définit par une atteinte organique et aiguë du parenchyme rénal en contexte de surdosage en anti-vitamine K (AVK) (international normalized ratio, INR > 3).ObservationNous rapportons l'observation d'un homme de 70 ans, dont la fonction rénale était antérieurement normale, et qui développait une WRN lors d'un surdosage franc. La récupération de la fonction rénale était très incomplète.ConclusionChez un patient présentant une insuffisance rénale aiguë dans un contexte de traitement par AVK, un INR > 3 doit faire envisager le diagnostic de WRN, surtout s'il existe une hématurie. Le diagnostic de certitude est histologique (congestion des tubules rénaux par des hématies). L'administration de vitamine K reste la seule stratégie thérapeutique reconnue pour cette entité de mauvais pronostic rénal et associée à une augmentation de la mortalité.

IntroductionWarfarin-related nephropathy (WRN) is a newly recognized entity, which is characterized by the occlusion of renal tubules by red blood cells following glomerular hemorrhage in a patient overexposed to warfarin (international normalized ratio > 3).Case reportWe report a 70-year-old man with no previous renal condition who developed WRN when his INR was > 12. He did not fully recover his previous renal function.ConclusionThe diagnosis of WRN should be considered whenever INR exceeds 3 in patients exposed to warfarin, particularly in the presence of hematuria. Vitamin K is the only therapeutic option.
