RiassuntoObiettiviDescrivere il trattamento endodontico di un incisivo centrale con due canali e caratterizzato da estese calcificazioni, mostrando come sia possibile affrontare e come possa essere complicato reperire un’anatomia complessa e senza riferimenti.Materiali e metodiTalvolta un radiogramma non è sufficiente a indirizzarci verso la soluzione di casi complessi, perché anche un’attenta analisi, in caso di estese calcificazioni, può essere insufficiente a interpretare correttamente l’anatomia canalare. È quindi indispensabile verificare clinicamente le condizioni dell’elemento dentale, se possibile avvalendosi di un microscopio che, grazie al forte fattore di ingrandimento, permette di apprezzare tutti i particolari che ci conducono all’identificazione di un’anatomia a volte non altrimenti riconoscibile. Abbinate al microscopio, le punte montate su ultrasuoni permettono di lavorare mantenendo una buona visibilità del campo operatorio. Reperiti i tragitti endodontici, l’elemento in questione è stato trattato secondo la tecnica crown-down e chiuso con guttaperca termoplasticizzata applicando l’onda continua di condensazione.Risultati e conclusioniNei casi complessi, oltre a prestare particolare attenzione alla fase diagnostica al fine di compilare un corretto piano di trattamento per l’elemento dentale interessato, è imprescindibile approcciare il caso clinico avvalendosi dei mezzi più idonei, quali il microscopio operatorio e le punte ultrasoniche: questi presidi, infatti, possono essere indispensabili per completare il trattamento endodontico di elementi molto calcificati.

ObjectivesTo describe the endodontic treatment of a central maxillary incisor with two root canals, characterized by wide calcifications, showing that it can be faced and that finding a complex anatomy without any reference can be very difficult.Materials and methodsSometimes, a radiograph is not enough to lead to the solution of complex cases, because in case of extensive calcifications it can be insufficient to correctly construe the root canal anatomy, even after an accurate analysis. It is then mandatory to clinically verify the conditions of the tooth, if possible using a microscope that with its strong magnification allows to appreciate all the features that lead us to identify an anatomy that cannot be otherwise recognized. Together with the microscope, ultrasonic tips allow to work keeping a good visibility of the operative field. Once the endodontic pathways were found, the tooth was treated according to the crown-down technique and filled with warm gutta percha (continuous wave of condensation).Result and conclusionsIn complex cases characterized by extended calcifications, it is useful to pay attention to the diagnostic phase in order to formulate a valid treatment plan for the tooth, and it is mandatory to consider the individual case report using the most suitable instruments, such as operative microscope and ultrasonic tips. Indeed, these instruments can be indispensable in order to finish the endodontic treatment of calcified teeth.
