Fistule congénitale médiane de la lèvre supérieure associée à un syndrome de Goldenhar : à propos d’un cas

RésuméBut de l’étudeNous présentons le cas d’une patiente présentant un syndrome de Goldenhar associé à une fistule congénitale située au milieu du philtrum au niveau de la lèvre supérieure.Patients et méthodeLa patiente a été prise en charge dès sa naissance pour un syndrome de Goldenhar. Plusieurs interventions ont été réalisées dans l’enfance afin de corriger une macrostomie droite et une hypoplasie mandibulaire droite. À l’âge de 14 ans, elle bénéficie d’une rhinoplastie pour corriger une malformation nasale complexe. En peropératoire un orifice situé dans l’espace inter-incisif médian supérieur est découvert. Il se prolonge par une fistule qui se dirige à travers la voûte palatine antérieure en direction du rail vomérien, à la base de la cloison. Cette fistule est borgne en arrière du vomer et peut être réséquée en totalité. L’examen anatomopathologique de la pièce révèle un revêtement malpighien.RésultatsLes fistules médianes de la lèvre supérieure sont extrêmement rares : moins de 30 cas reportés dans la littérature. Le trajet inhabituel de la fistule et l’apparition récente d’un syndrome hypophysaire chez cette patiente nous a fait rechercher une continuité entre celle-ci et la poche de Rathke par une imagerie de type tomodensitométrie.ConclusionL’association d’un syndrome de Goldenhar et d’une fistule congénitale médiane de la lèvre supérieure n’a jamais encore été décrite dans la littérature. L’association à un hypopituitarisme a fait suspecter un prolongement postérieur de la malformation, non confirmé par le scanner, mais fortement présumé conjointement par l’équipe de neurochirurgie et de chirurgie plastique.

SummaryAim of the studyWe present the case of a patient with Goldenhar syndrome associated with congenital fistula in the middle of philtrum at the upper lip.Patients and methodsThe patient was supported from birth for Goldenhar syndrome. Several procedures were performed in childhood to correct a macrostomia and mandibular hypoplasia right. At 14 years old, she has a rhinoplasty to correct a complex nasal malformation. During the procedure, a hole in the upper central incisor inter-space is found. It is extended by a fistula which runs through the front palate towards the vomer rail at the base of the partition. This fistula is blind behind the vomer and can be completely resected. Pathological examination of the resection reveals a squamous lining.ResultsThe median fistula of the upper lip are extremely rare : less than 30 cases reported in the literature. The unusual run of the fistula and the recent appearance of a pituitary syndrome in this patient makes us look for a continuity between it and Rathke's pocket by computed tomography imaging type.ConclusionThe association of Goldenhar syndrome and median congenital fistula of the upper lip was never yet described in the literature. The association with hypopituitarism was suspected with the posterior extension of the malformation, not confirmed by the scanner, but strongly suspected jointly by the neurosurgical and plastic surgery team.
