Évaluation d’un dispositif d’annonce et d’information adapté à la reconstruction mammaire en cancérologie

RésuméButÉvaluer la mise en place d’une consultation infirmière et d’un support DVD interactif en reconstruction mammaire. Juger de l’utilité de ces nouveaux outils d’information pour les malades.Matériels et méthodesLa consultation infirmière est réalisée après la consultation initiale du chirurgien. Lors de cette consultation, est remis un DVD contenant des interviews reprenant les informations données ainsi que des témoignages de patientes sur leur parcours de soin. Évaluation de l’information donnée par le chirurgien et par l’infirmière (huit items allant des techniques chirurgicales à la convalescence). Évaluation de l’intérêt et de la compréhensibilité. Cotation de 0 à 10.RésultatsÉvaluation par questionnaire anonyme sur 110 femmes prises en charge entre février et décembre 2007. Taux de participation : 72,7 % (80/110), âge moyen : 51,5 ± 9,2 ans. L’information donnée est jugée utile (chirurgien : note supérieure à 9,3/10 en fonction des items, infirmière : note supérieure à 9,4) et compréhensible (note supérieure à 8,7/note supérieure à 8,9). L’information faite par l’infirmière est plus compréhensible que celle du chirurgien, quels que soient les items. La consultation infirmière est jugée très utile (9,45). Soixante-dix pour cent des patientes ont appris des informations supplémentaires, par rapport à la consultation du chirurgien. Cette consultation n’a pas d’influence directe sur le choix de la technique (5,45/10), la consultation du chirurgien restant prépondérante. Cinquante pour cent des femmes ont encore appris des éléments nouveaux grâce au DVD. En revanche, il n’a peu d’influence sur le choix de la technique (4,84/10). La consultation infirmière et le DVD sont bien conçus car 80 % des femmes n’ont pas eu besoin de plus d’information. Les éléments prépondérants dans le choix des malades sont la consultation du chirurgien, puis la consultation infirmière, puis le DVD. L’indice global de satisfaction du dispositif mis en place est très bon 9,31/10 (chirurgien : 9,24 ; infirmière 9,56, DVD 9,11).ConclusionLa consultation infirmière et le DVD ne sont pas des éléments de substitution de la consultation du chirurgien. Ils sont complémentaires et ont un rôle important dans l’acquisition de l’information et le soutien psychologique des malades. La consultation initiale du chirurgien reste prépondérante dans la décision de choix par les malades, elle doit rester la plus complète possible.

The predominant elements in the choice of the patients are the surgeon consultation, then the nurse consultation and then the DVD. The total index of satisfaction is very good 9.31/10 (surgeon: 9.24, nurse 9.56, DVD 9.11).ConclusionThe nurse consultation and the DVD are not replacement components of the surgeon consultation. They are complementary and have an important role in the acquisition of information and the psychological support of the patients. The surgeon initial consultation remains predominent in the decision of choice by the patients, it must remain the most complete possible.
