Influence of topical bimatoprost on macular thickness and volume in glaucoma patients with phakic eyes 1

Background: In light of concern about the possible relation between the extensive clinical use of prostaglandin analogues and the development of retinal disorders such as cystoid macular edema, this study investigated the influence of topical application of bimatoprost 0.03% on macular thickness and volume in glaucoma patients with phakic eyes.Methods: A total of 40 eyes in 22 patients with primary open-angle glaucoma or normal-tension glaucoma were evaluated in this study. Optical coherence tomography images were taken before initiation of bimatoprost application and after 1, 2, and 3 months of treatment. In addition, best-corrected visual acuity (BCVA) and intraocular pressure (IOP) were measured, and contrast sensitivity tests and fundus examinations were performed. Changes in various parameters, including macular thickness and volume, were analyzed.Results: BCVA and contrast sensitivity did not change from baseline over the treatment period. IOP declined by 20% to 25% from baseline for 1 month before stabilizing. Macular thickness and volume did not increase significantly during the 3 months.Interpretation: Topical application of bimatoprost does not induce clinical or structural changes in the macula, at least in glaucoma patients with phakic eyes. Moreover, bimatoprost effectively lowered IOP without causing macular disorders such as cystoid macular edema.

RésuméContexte: Vu l’inquiétude que soulève la possibilité d’un rapport entre l’usage clinique étendu des analogues de la prostaglandine et le développement de troubles rétiniens comme l’œdème maculaire cystoïde, cette investigation porte sur l’effet de l’application topique de bimatoprost 0,03% sur l’épaisseur et le volume de la macula chez les patients glaucomateux avec yeux phakiques.Méthodes: En tout, 40 yeux de 22 patients atteints de glaucome primaire à angle ouvert ou de glaucome normotensif ont fait l’objet de l’évaluation. On a fait des tomographies en cohérence optique avant l’administration du bimatoprost et 1, 2 et 3 mois après le traitement. En outre, on a mesuré la meilleure acuité visuelle corrigée (MAVC) et la pression intraoculaire (PIO), effectué les tests de sensibilité au contraste, examiné le fond d’œil et analysé l’épaisseur et le volume de la macula.Résultats: La MAVC et la sensibilité au contraste n'ont pas changé du début à la fin du traitement. La PIO a baissé de 20% à 25% dans le premier mois et s'est ensuite stabilisée. L’épaisseur et le volume de la macula n'ont pas augmenté de façon significative pendant les 3 mois.Interprétation: L’application topique du bimatoprost n'entraîne pas de changements cliniques ni structurels dans la macula, du moins chez les patients glaucomateux avec yeux phakiques. De plus, le bimatoprost a abaissé efficacement la PIO sans causer de troubles maculaires comme l’œdème maculaire cystoïde.
