Embolisation par voie artérielle de malformations artérioveineuses durales intracrâniennes à l’aide d’éthylvinyl alcool copolymère (Onyx®18) : à propos de trois observations

RésuméObjectifsRapporter trois observations de malformations artérioveineuses durales (MAVD) intracrâniennes, traitées par injection transartérielle d’éthylvinyl alcool copolymère (Onyx®18).Patients et méthodesNous avons analysé les résultats cliniques et angiographiques chez trois patients porteurs d’une MAVD révélée par une complication hémorragique et traitée par embolisation à l’aide d’Onyx. L’exploration angiographique a montré une MAVD localisée au niveau du sinus latéral chez deux patients et de la tente du cervelet chez le troisième.RésultatsL’embolisation a été réalisée par cathétérisme sélectif du pédicule artériel méningé alimentant la MAVD et injection unique d’Onyx®. Un contrôle angiographique réalisé en fin de traitement a confirmé l’exclusion anatomique complète de ces trois MAVD. Cliniquement, les patients ont récupéré de façon partielle ou totale leur déficit neurologique.ConclusionL’Onyx® est un agent embolique récemment introduit pour le traitement des MAVD intracrâniennes. L’utilisation de ce produit dans des conditions optimales, offre des options thérapeutiques intéressantes dans le traitement de ces malformations.

SummaryBackgroundThis is a report of the endovascular treatment of three intracranial dural arteriovenous malformations (DAVM) using Onyx®.Patients and methodsWe analyzed the clinical and angiographic results in three patients with intracranial DAVM, revealed by a hemorrhagic event and treated by endovascular way by the use Onyx. Angiographic investigation showed the DAVM to be located in the lateral sinus in two cases and in the tentorium region in the third.ResultsIn all three cases, embolization was performed by selective catheterization of the meningeal arterial feeder of the DAVM. The injection of Onyx® resulted in complete anatomical exclusion of the DAVM, as demonstrated by posttreatment angiography. Clinically, the patients recovered partially or completely their neurological deficit.ConclusionOnyx® is a liquid embolization agent recently introduced for the treatment of DAVM. When used under optimal conditions, it offers a feasible alternative option in the treatment of such vascular malformations.
