PET/CT in radiotherapy planning in head and neck cancers

18F-FDG-PET-CT has showed its utility in the staging and the definition of the treatment volume in head and neck cancers with radiotherapy (RT) indication. Despite some uncertainties related to resolution and segmentation, PET/CT offers valuable information about tumor heterogeneity and biology. The technological advances in the radiotherapy setting as intensity-modulated RT, image-guided RT in the adaptive protocol and dose escalation are the future directions for the PET-CT especially for hypoxia or proliferation radiotracers.

RésuméLa TEP/TDM au 18FDG a montré son utilité dans la stadification et la définition du volume tumoral traité en radiothérapie pour les cancers de la tête et du cou. Malgré quelques incertitudes relatives à la résolution et à la segmentation, la TEP/TDM offre des informations valables concernant l’hétérogénéité et la biologie tumorales. Les avancées technologiques en radiothérapie, comme la radiothérapie avec modulation d’intensité (IMRT), la radiothérapie guidée par l’image (IGRT) dans les protocoles de radiothérapie adaptative et les escalades de dose, sont les directions futures de la TEP/TDM spécialement pour l’hypoxie et les radiotraceurs de prolifération.
