RésuméL’objectif de cette étude est d’évaluer les gains éventuels de spécificité (Sp) et de sensibilité (Se) apportés par la TEMP/TDM axiale 3 champs de vue (FOV) sur la scintigraphie osseuse de balayage corps entier (SOCE) dans les indications oncologiques et/ou dans le diagnostic des lombalgies d’origine indéterminée.Patients et méthodesÉtude rétrospective comparant les images SOCE et TEMP/TDM 3 FOV chez 39 patients (27 hommes, 12 femmes, âge : 65 ± 14 ans [moyenne ± écart-type]). L’étalon de vérité correspond à la biopsie osseuse et/ou aux autres modalités d’imagerie métabolique (TEMP/TDM au pentetréotide, TEP/TDM au FDG et/ou FNa, nouvelle scintigraphie osseuse) et morphologique (TDM diagnostique, IRM).RésultatsLa SOCE a décelé 409 lésions dont 252 malignes, 31 bénignes et 126 de nature indéterminée. La TEMP/TDM a détecté 438 lésions dont 391 malignes, 31 bénignes et 16 de nature indéterminée. Les Se, Sp, valeurs prédictives positives (VPP) et négatives (VPN) de la SOCE et de la TEMP/TDM sont respectivement 91 %, 58 %, 94 %, 48 % et 100 %, 87 %, 98 %, 100 % (différences significatives). Nous avons comparé la Se et la Sp de la TEMP/TDM, de l’IRM pelvirachidienne et de la TDM TAP pour les lésions rachidiennes chez 24 patients. Les Se et Sp sont respectivement 98 %, 81 %, 100 % et 69 %, 100 %, 57 % (différences non significatives).ConclusionLes performances diagnostiques de la TEMP/TDM associées à l’argumentaire physiopathologique, épidémiologique et clinique conduisent à envisager en routine oncologique clinique la TEMP/TDM 3 FOV sans balayage corps entier, au prix d’un risque théorique exceptionnel de faux-négatifs.
The objective of this study is to assess the potential gain of specificity and sensitivity brought by 3 field-of-view (3-FOV) SPECT-CT modality covering axial skeleton over whole body scintigraphy (WBS) and SPECT-CT in an oncologic setting and/or in the work-up of low back pain of obscure origin.Patients and methodsRetrospective study comparing the WBS and 3-FOV SPECT-CT images in 39 patients (27 males, 12 females, age: 65 ± 14 years [mean ± standard deviation]). The criterion standard is composite and comprehends bone biopsy and/or metabolic imaging (pentetreotide SPECT-CT, FDG and/or NaF PET-CT, new bone scintigraphy) and morphologic imaging (MDCT, MRI) modalities. The results are analysed statistically by 95% confidence interval (95% CI).ResultsThe whole body scintigraphy (WBS) detected 409 lesions, including 252 malignant, 31 benign, and 126 indeterminate ones. On the other hand, 438 lesions were detected by the 3-FOV SPECT-CT, including 391 malignant, 31 benign, and 16 indeterminate ones. Lesionwise Se of WBS and SPECT-CT was 91% and 100% respectively. Sp was 58% and 87% respectively. The PPV was 94% and 98% respectively. NVP was 48% and 100% respectively (differences all significant). We compared lesionwise Se and Sp of 3-FOV SPECT-CT, pelvic and spinal MRI, chest-abdomen-pelvis CT of the vertebral lesions in 24 patients. Se and Sp were 98%, 100% and 100% respectively and 81%, 69% and 57% respectively (differences not significant).ConclusionThe constellation of pathophysiological, epidemiological, clinical and technical arguments contributes to consider the 3-FOV SPECT-CT unassociated to whole body scintigraphy, in clinical oncologic routine, at the cost of an exceptional theoretical risk of false negative.
