Fixation inhabituelle au balayage à l’iode-131 : un textilome !

RésuméMalgré ses bonnes sensibilité et spécificité, la scintigraphie corps entier à l’iode-131(131I) peut présenter de nombreux faux-positifs. Ces artefacts peuvent compromettre la bonne interprétation de l’examen. Le but de ce rapport de cas est d’illustrer une autre origine de faux-positifs au radioiode, jusque-là non encore rapportée. Il s’agit d’une patiente suivie pour carcinome vésiculaire de la thyroïde opéré et qui a présenté une masse clinique et échographique simulant une adénopathie cervicale. Le balayage réalisé huit jours après administration de dose thérapeutique d’131I a mis en évidence une fixation du traceur au niveau de cette masse. Cependant, après réintervention, nous avons été surpris d’être devant un corps étranger : un textilome. Dans cet article et à la lumière des écrits rapportés à ce sujet, nous essayons de discuter les hypothèses physiopathologiques pouvant expliquer la fixation de l’131I au niveau du corps étranger.

Despite its good sensitivity and specificity, the whole body scintigraphy with iodine-131 (131I) may present many false-positives. These artefacts can compromise the correct interpretation of the examination. The purpose of this case report is to illustrate another source of false-positives with radioiodine, so far not yet reported. It is the case of a patient followed for operated follicular thyroid carcinoma and who presented a mass simulating a cervical lymphadenopathy. The scan performed eight days after administration of therapeutic dose of 131I revealed the tracer uptake at this mass. However, after reoperation, we were surprised to be in front of a foreign body: a textiloma. In this paper, and in light of literature, we try to discuss the pathophysiological hypotheses that may explain the uptake of 131I at the foreign body.
