RésuméButUne étude de phase II promue par le GOELAMS, multicentrique, non randomisée, est en cours, évaluant l’efficacité et la tolérance de l’administration fractionnée de l’90Y-epratuzumab (deux injections à sept jours d’intervalle), dans le traitement de consolidation de patients de plus de 60 ans atteints de lymphomes diffus à grandes cellules de forte masse tumorale, en réponse complète (RC), RC non confirmée (RCu) ou réponse partielle (RP) après un traitement d’induction comportant six cures de R-CHOP14. Nous rapportons des résultats préliminaires chez 41 des 75 patients inclus et en cours de suivi.Patients et méthodesLa toxicité était évaluée selon des critères NCI-CTC version 3. Le taux de RC et de RCu ainsi que le taux de conversion des RP en RC/RCu six semaines après les injections de 90Y-epratuzumab ont été évalués selon les critères IWRC.RésultatsTrente-trois patients ont reçu les deux injections d’90Y-epratuzumab. La toxicité de l’90Y-epratuzumab était essentiellement hématologique, caractérisée par une neutropénie grade 3/4 chez 81,8 % des patients et une thrombopénie grade 3/4 chez 78,8 % des patients, réversibles. Deux patients (6,1 %) ont présenté une toxicité non hématologique de grade 3/4. Six semaines après les injections de 90Y-epratuzumab, 40,0 % des patients amélioraient leur réponse et le taux de RC/RCu était de 83,9 %.ConclusionCes résultats préliminaires montrent une toxicité importante mais essentiellement hématologique, attendue en consolidation. Le taux de réponse apparaît encourageant et reste à confirmer par un nombre d’inclusions et un recul suffisants.
PurposeA multicenter, non-randomized, on-going phase II study, promoted by the GOELAMS aims to evaluate the efficacy and safety of fractionated administration of 90Y-epratuzumab (two injections 7 days apart) in consolidation therapy for patients older than 60 years with diffuse large B-cell lymphoma and large tumor mass who presented a complete response (CR), unconfirmed CR (uCR) or partial response (PR) after induction therapy with six courses of R-CHOP14. We report the preliminary results on 41 of the 75 enrolled patients.Patients and methodToxicity was evaluated according to NCI-CTC version 3. The CR and uCR rate and the rate of conversion of PR to CR/uCR 6 weeks after injections of 90Y-epratuzumab were evaluated using IWRC criteria.ResultsThirty-three patients received two injections of 90Y-epratuzumab. The toxicity of 90Y-epratuzumab was mainly hematologic, characterized by neutropenia grade 3/4 in 81.8% of patients and thrombocytopenia grade 3/4 in 78.8% of patients, reversible. Two patients (6.1%) had grade 3/4 non-hematologic toxicity. Six weeks after injection of 90Y-epratuzumab, 40.0% of patients improved their response and the rate of CR/RCu was 83.9%.ConclusionThese preliminary results show significant toxicity, but mainly hematologic, expected in consolidation. The response rate is encouraging and needs to be confirmed by sufficient number of inclusions and follow-up.
