Absence de fixation du gril costal: Diagnostic d’un syndrome de Poland par la TEMP-TDM osseuse

RésuméNous rapportons le cas d’un homme de 37 ans, exploré pour douleurs du membre supérieur droit touchant la main, le poignet et l’épaule après une chirurgie pour fracture du radius et du scaphoïde. La scintigraphie osseuse dynamique au 99mTc-HDP a confirmé le diagnostic d’algodystrophie chaude. Les images tardives ont mis en évidence une absence de fixation localisée du gril costal antérieur gauche. La tomographie d’émission monophotonique couplée à la tomodensitométrie a montré qu’elle était liée à l’absence de l’arc antérieur de la troisième et de la quatrième côtes gauches et a mis en évidence une aplasie du muscle pectoralis major gauche. Ces anomalies ont conduit au diagnostic de malformation congénitale en rapport avec un syndrome de Poland. Le diagnostic différentiel de ce syndrome a fait discuter les multiples étiologies des lésions photopéniques du gril costal.

We report the case of a 37-year-old man, investigated for pains of the right upper limb impairing the hand, the wrist and the shoulder after a surgery for fracture of the radius and the scaphoid. The 99mTc-HDP three-phase bone scintigraphy confirmed the diagnosis of acute phase of a reflex sympathetic dystrophy. The delayed images highlighted a focal absence of tracer uptake of the left anterior rib cage. The single photon emission computerized tomography guided by computerized tomography showed that it was related to the absence of the anterior part of the third and the fourth left ribs highlighting an aplasia of the left pectoralis major muscle. These anomalies led to the diagnosis of congenital malformation in connection with a Poland's syndrome. The differential diagnosis of this syndrome resulted in discussing the multiple etiologies of the photopenic lesions of the rib cage.
