► We searched all electronic databases and manual references to find relevant articles. ► Quality assessment, data extraction, and meta-analysis of eligible studies were done. ► Regulatory T cells lead to chronicity, high viral load, and hepatic carcinoma (HCC). ► Regulatory T cells are associated with no response to therapy and immunosuppression. ► Regulatory T cells are therapeutic target for immunotherapy of HBV infected patients.
