The Validation of a New System of Transcription of Acupuncture Points on the Ear: the Auricular Sectogram∗

IntroductionA reliable system of transcription of the points mapped on the outer ear is essential for auricular diagnosis. Moreover the correct transcription and recording of these points helps the physician to understand if a patient's condition is going to improve with therapy. Several bi-dimensional graphic systems have been proposed in the past for transcribing the auricular points.ObjectiveTo validate a new system of transcription of points: the Auricular Sectogram.MethodsAfter attending a theoretical-practical course of auricular diagnosis and therapy 385 physicians were asked to participate in the following transcription exercise: two sets of points, I and II, were projected onto a screen one after the other. Each set was composed of 3 points located on 3 different parts of the pinna (helix, anthelix and earlobe).The physicians were asked to transcribe the points of sets I and II in random order on the Sectogram or on a map of the auricle without subdivision into sectors. These two modalities were compared for total average number of correct transcriptions as well as for their relative proportions on the different parts of the ear. The same procedure was adopted with the average total number of correctly identified sectors and their proportions.ResultsThe total average number of correctly transcribed points was significantly higher using the Sectogram (p < 0.001) compared to an auricular map without subdivision into sectors. Also the proportions of points correctly transcribed on the different parts of the ear (helix, anthelix and earlobe) were significantly different. The same results were found comparing the total number of correctly identified sectors (p < 0.001) and their proportions on the 3 different parts of the ear.ConclusionsThe Auricular Sectogram allows practitioners to transcribe more precisely the site of auricular points and areas for diagnostic and therapeutic purposes

ZusammenfassungEinleitungEin verlässliches Transkriptionssystem der Punkte der Ohrmuschel ist entscheidend für die aurikulare Diagnostik. Des Weiteren hilft die korrekte Transkription und Eintragung dieser Punkte dem Arzt bei der Einschätzung, ob der Patient von der Therapie profitiert. Verschiedene zweidimensionale grafische Systeme zur Transkription der Ohrakupunkturpunkte sind bereits in der Vergangenheit vorgeschlagen worden.ZielDie Validierung eines neuen Systems der Punkttranskription: das aurikulare Sektogramm.MethodikNach der Teilnahme an einem theoretischen und praktischen Kurs in Aurikulardiagnostik und -therapie nahmen 385 Ärzte an der folgenden Transkriptionsübung teil: zwei Punktgruppen (I und II) wurden, eine nach der anderen, auf einen Bildschirm projiziert. Jede Gruppe bestand aus jeweils drei Punkten auf drei verschiedenen Teilen der Ohrmuschel (Helix, Anthelix und Ohrläppchen). Die Aufgabe der Ärzte war nun, die Punkte der Gruppen I und II in ungeordneter Reihenfolge auf das Sektogramm oder eine Darstellung der Ohrmuschel ohne Unterteilung in Sektoren zu übertragen. Die beiden Modelle wurden hinsichtlich der durchschnittlichen korrekt übertragenen Punkte verglichen sowie bezüglich der relativen Proportionen auf den verschiedenen Teilen der Ohrmuschel. Das gleiche Vorgehen wurde für die durchschnittliche Gesamtzahl der korrekt identifizierten Sektoren und ihrer Proportionen angewendet.ErgebnisseDie durchschnittliche Gesamtzahl der korrekt übertragenen Punkte war bei Verwendung des Sektogramms signifikant höher (p < 0.001) verglichen mit der Darstellung einer Ohrmuschel ohne Unterteilung in Sektoren. Des Weiteren bestand ein signifikanter Unterschied hinsichtlich der Proportionen der korrekt übertragenen Punkte auf den verschiedenen Teilen der Ohrmuschel (Helix, Anthelix und Ohrläppchen). Der Vergleich der Gesamtzahl der korrekt identifizierten Sektoren (p < 0.001) und ihrer Proportionen auf den drei Teilen der Ohrmuschel lieferte ähnliche Ergebnisse.SchlussfolgerungDas aurikulare Sektogramm erlaubt dem Akupunkteur eine präzise Transkription der Ohrakupunkturpunkte und -zonen, die für diagnostische und therapeutische Zwecke verwendet werden.
