Psychophysiologische Wirkmechanismen von Akupunktur in der Behandlung von Schmerzen

ZusammenfassungEs werden neurobiologische Wirkmechanismen, gegliedert in lokale und systemische Effekte, sowie psychologische Wirkelemente der Akupunktur in der Behandlung von Schmerzen erörtert. Akupunktur wird hierbei als repetitive nociceptive Reiztherapie aufgefasst, die durch die Induktion von Adaptationsprozessen auf verschiedenen Ebenen des Organismus zu einer Aktivierung körpereigener Schmerz hemmender Mechanismen führt. Die Betrachtung der psychophysiologischen Wirkungen ergänzt das klassische Konzept der Akupunkturtherapie und bietet dem Therapeuten eine zusätzliche Orientierungshilfe, den Blick auf die relevanten Aspekte der Therapie zu fokussieren. Es ist zu erwarten, dass im Zuge einer weiteren Erforschung wirkungsrelevanter Faktoren der Akupunktur, diese zur Optimierung des Therapieerfolges gezielter eingesetzt werden können.

This review explores neurobiological and psychological mechanisms of acupuncture in the treatment of pain. Data are critically evaluated regarding their relevance for clinical practice. Acupuncture, in this context, is considered as a mode of repetitive, nociceptive stimulation which induces adaptive processes on different levels of the organism leading to an activation of nocifensive mechanisms that improve the ability to cope with pain. The knowledge of the psychophysiological mechanisms complements the classical Chinese approach and offers an additional possibility for the therapist to focus on relevant aspects of the treatment. It is supposed that in the course of further scientific exploration of the mechanisms of action, these might be evoked in a more differentiated way in acupuncture treatment.
