► We review instruments for patient-reported adverse effects of antiepileptic drugs. ► Strengths, limitations and psychometric properties of 8 instruments are discussed. ► Mood factors, depression and anxiety can influence self-report of adverse effects. ► Epilepsy care can be improved by routine use of adverse-effects questionnaires.
