Découverte « fortuite » d'un syndrome d'apnées obstructives du sommeil au cours d'un bilan d'hypertension pulmonaire suspectée par échocardiographie

The discovery of pulmonary hypertension (PH) is a common event in the setting of obstructive sleep apnoea syndrome (OSAS). The discovery of OSAS during the investigation of PH is conversely rare, especially if the OSAS is associated with no or with few symptoms. We report the case of a 64-year-old patient referred by his cardiologist for a diagnostic work-up of PH suspected by echocardiography during follow up of his essential hypertension. The patient did not complain of snoring or daytime sleepiness. Ventilatory polygraphy was performed systematically and enabled a diagnosis of severe OSAS. Continuous positive airway pressure (CPAP) was well tolerated and pulmonary blood pressure normalised. This article discusses an unusual case of OSAS revealed by PH.
