Treating neuropathic pain remains a great challenge. Recommended first-line treatments include tricyclic antidepressants and alpha-2-delta agonists for multiple neuropathic situations, the dual reuptake inhibitors of both serotonin and norepinephrine in diabetic neuropathy, and lidocaine patches for post-herpetic neuralgia. Therapeutic prospects include focal therapy with sustained efficacy (i.e. capsaicine patches, botulinum toxin). The transcranial magnetic stimulation is a therapy with some evidence for efficacy. The methodology of clinical trials now takes better account of the sensory neuropathic symptoms of patients, which should contribute to better prediction of responders to treatment.
