Un infarctus cérébral révélant un myélome multiple

Haematological disorders are rare causes of cerebral infarction. We report a case of cerebral infarction revealing a multiple myeloma. The patient was a 76-year-old female who was admitted with right hemiparesis and aphasia of sudden onset. A brain scan showed early ischemic signs. The bone window evidenced numerous skull geodes. The rib cage showed lysis of the right clavicle. Serum protein electrophoresis evidenced monoclonal hypergammaglobulinemia of 59.5 g/L. The myelogram showed a 71% medullary invasion by dysmorphic plasmocytes. The occurrence of stroke in multiple myeloma is extremely uncommon, however, it can sometimes be observed as an initial presentation or in the course of evolution of the disease.
