La transformation du côté du thérapeute comme préalable au changement du patient en situation transculturelle

RésuméLes effets des différences culturelles entre patient et thérapeute, repérés dans un dispositif de consultation transculturelle, interfèrent tant sur l’initialisation du processus thérapeutique que sur les processus de changement qu’ils contribuent à éclairer de manière spécifique. Le dispositif d’entretien thérapeute/interprète favorise une dynamique thérapeutique renforcée par les enjeux du travail sur la langue. Les effets contre-transférentiels de cette pratique repérés à partir de deux situations cliniques mettent en évidence comment la transformation du matériel psychique du patient passe par une transformation préalable du côté du thérapeute portant sur son fonctionnement psychique et prenant pour cela appui sur le groupe d’interprètes. L’auteur reconnaît trois niveaux de temporalité dans cette dynamique de changement en situation transculturelle.

Cultural differences between patient and therapist, apparent during cross-cultural consultations, turn out to influence as much the origins of the therapeutic process as the ensuing changes they contribute to highlighting in a specific manner.The therapist/interpreter interaction favours the dynamics of a therapeutic process reinforced by the attention to language and its stakes. Two clinical situations illustrate the countertransferential effect of such practices by revealing how the change brought about in the material of the patient's psyche is conditioned by a prior change within the therapist's psyche, the latter being supported by the group of interpreters. According to the author, this process of change in a cross-cultural situation is experienced along three different temporal modulations.
