Diagnostic imaging plays an important role in the management of patients with hepatocellular carcinoma (HCC) since pathology confirmation is not mandatory for definitive diagnosis. The Liver Imaging Reporting and Data System (LI-RADS) is a system for standardized reporting of liver imaging examinations in patients at risk for HCC. This system may be used by all radiologists, academic or community physicians providing care to patient with liver disease. LI-RADS assigns to each observation - defined as an area with imaging features that differ from those of adjacent liver parenchyma - a category reflecting his probability of HCC. This system was designed to facilitate communication between health professionals involved in the care of patients at risk for HCC, to promote education and research. LI-RADS is a dynamic system iteratively refined with feedback from users. The purpose of this educational article is to introduce LI-RADS version 2017, define LI-RADS categories, explain LI-RADS computerized tomography and magnetic resonance imaging diagnostic algorithm and table for categorization of observations, define major and ancillary imaging features to assign categories, and emphasize the potential benefits of LI-RADS application.
