Soigner la psychose. Le cadre de soin extrahospitalier : un objet à utiliser

RésuméCet article se propose d’interroger, à travers l’évocation d’une relation transférentielle forte vécue par l’auteur avec un patient psychotique, les effets d’un cadre de soin extrahospitalier, organisé sur le modèle d’un club thérapeutique. Ce dernier avait été pensé, au temps du mouvement de la Psychothérapie Institutionnelle, comme une pièce maîtresse du dispositif de transformation de l’institution psychiatrique. La fréquentation libre d’un tel lieu, c’est-à-dire hors de toute injonction thérapeutique, lui permettant, à notre avis, de s’inscrire dans une dialectique d’utilisation de l’objet au sens posé par Winnicott, avec le regard du soignant comme témoin de cette initiation. L’analyse fait saillir une dynamique d’investissement mettant en jeu le dedans et le dehors permettant de construire, détruire, reconstruire le cadre vers sa reconnaissance en tant qu’objet. L’hypothèse étant que ce processus serait susceptible de faire ré-entrer le patient psychotique dans un mouvement libidinal de subjectivation progressive par cette interaction particulièrement orchestrée avec l’objet cadre. L’attention vigilante du soignant, corollaire à une nécessité de réparation interne, conduit à une contenance émergeante du dispositif et fonctionne comme un élément central dans la possibilité de ré-individuation offerte au patient, en tant que révélateur de ce processus d’utilisation.

The present article examines, through the evocation of a transference relationship between the author and a psychotic patient, the effects of an outcare setting based on the model of a Therapeutic Club. This structure was created during the institutional psychotherapy's era and was seen as a fundamental way of transforming the psychiatric institution. The personal decision of the patient to attend this kind of specific structure, i.e. without the interference of therapeutic injunction but with the nursing staff notifying this initiative, allows, in our opinion, to make a direct link with Winnicott's use of an object theory. This analysis puts emphasis on the dynamics articulating the inside and the outside and allowing thus to build, destroy, rebuild the outer setting towards the identification as object. The main hypothesis is that the process would help the psychotic patient towards re-entering into a libinal and progressive subjectivation by orchestrating an interaction between the patient and the object. The extra-care of the nursing staff, corollary with an internal need of reparation, leads to the emergence of a containing function. It works like a central component for the patient in the progress of self-individualization, in terms of indicator of his use of an object process.
