Distress at end of life may be partly explained by the effect of breaking identity feelings led by changes in the body. Alterations in patient's identity resonate in the intersubjective space of care and can then have an impact on caregivers. These caregivers might often answer by defending themselves. Sometimes, when they have enough time, they might think about their own feelings and thus succeed to adapt their behavior using productive defenses. These motions allow necessary repositioning from caregivers to accommodate with the patient's crisis of dying. We offer through a short clinical report, reflections on patients' distress at end of life and opportunities for caregivers' answers.
