Les enveloppements humides initialement froids (packings) sont efficaces dans les troubles graves du comportement chez les enfants et adolescents autistes

RésuméAucun médicament ne permet à l’heure actuelle de traiter les symptômes centraux de l’autisme (anomalies du langage, troubles de la communication et de la socialisation, intérêts restreints), mais certains médicaments sont parfois utiles pour traiter les troubles graves du comportement. Les packings présentent également un intérêt clinique au sein de la prise en charge pluridisciplinaire des enfants et adolescents autistes les plus gravement souffrants, notamment en cas d’automutilations rebelles aux psychotropes habituels. Cette technique consiste en l’enveloppement de la personne en sous-vêtements dans des serviettes mouillées initialement froides (de + 10 à  + 15 °C) pendant environ 45 minutes plusieurs fois dans la semaine. Le patient est entouré de couvertures afin de faciliter le réchauffement et la détente musculaire du corps. Le patient est toujours accompagné de trois ou quatre personnes très attentives à la relation permise par l’enveloppement et le réchauffement qui suit le saisissement initial par le froid. Nous présentons ici une étude pilote concernant dix patients, ainsi que le plan d’une étude prospective multicentrique nationale randomisée contrôlée (essai thérapeutique de phase II concernant 162 patients répartis en trois groupes : rispéridone seule, rispéridone avec enveloppements secs, rispéridone avec enveloppements humides initialement froids). Pour pouvoir discuter et comparer les résultats, la méthodologie retenue (critères d’inclusion et échelles de mesure des troubles du comportement) est semblable à celle qui a vérifié l’efficacité de la rispéridone dans la prise en charge des troubles graves du comportement chez les enfants présentant un trouble envahissant du développement (TED). Nous attendons de cette étude qu’elle aide à mieux préciser les indications thérapeutiques des packings pour mieux prendre en charge les enfants autistes.

Although there is currently no drug available to treat the core problems of autistic condition, non-specific drugs, mainly antipsychotics or neuroleptics, along with specialized educational and therapeutic programs, behavioral therapy and environmental changes, have been recognized to improve aggressive behavior of patients with autism. The practice of “packing”, as part of the therapeutic approach of patients with autism and serious behavioral problems may be of great interest for children and adolescents with autistic disorder, especially in the case of self-injurious behavior, and who do not respond to usual psychotropic drugs or who cannot tolerate them. This method consists in wrapping the patient with his under-clothes several times a week, using towels previously wet in cold water (+ 10 to + 15 °C). The patient is wrapped with blankets to help the body warm up. The envelopment lasts 45 minutes. Three or four members of staff are always looking after the patient and are carefully attentive to the quality of the relation they experience with the patient. The wrapping and the warming up, which follow the initial chill, seem to help the patient achieve a greater perception and integration of his body, allowing him to experience a growing sense of containment. We here present the results of a pilot-study about 10 cases, and introduce a randomized, blind and controlled study. We have chosen the same methodology (eligibility criteria and behavioral disorders rating scales) used to verify the effectiveness of risperidone in children with pervasive developmental disorders suffering of severe behavioral disorders. This study should help us specify the therapeutic indications of packing in autism.
