RésuméL'anorexie mentale de l'enfant et de l'adolescent est reconnue comme une pathologie d'étiologie multifactorielle. Malgré une meilleure compréhension étiopathogénique, la mortalité est encore importante. L'objet de cet article est de retranscrire une expérience clinique hospitalière auprès d'adolescents anorectiques au moment de la mise en jeu du pronostic vital. À partir d'une observation clinique, nous montrons que la compréhension psychodynamique du trouble est alors un outil important pour la construction de la conduite thérapeutique. Nous proposons ensuite une réflexion théorique à partir du concept du négatif et décrivons les conséquences pratiques qui en ont découlé.
Child and adolescent anorexia nervosa is now recognised as multi-etiological pathology. Despite the level of understanding, death rate is still important. The aim of this paper is to describe clinical experience with anorectic adolescents when the risk of death is extremely strong. Through the description of a case report we underline the importance of psychodynamic understanding. It is an helpful instrument in the choice of therapeutical interventions. We present a theorical point of view and the subsequent practical results.
