Le sens des thérapeutiques psychomotrices en psychiatrie de l'enfant

RésuméDu bébé à l'adolescent, de l'affect à la représentation, du symptôme le plus instrumental au syndrome le plus envahissant, du paradigme autistique au paradigme instable, en passant par nombre de problématiques psychopathologiques plus « limites » ou dysharmoniques, de nombreuses questions théoriques d'envergure, des interrogations psychopathologiques princeps (dynamiques et développementales à la fois) et combien de problématiques techniques et thérapeutiques essentielles s'entrecroisent au lieu même des expériences du corps-en-relation dont la sphère psychomotrice cerne une grande part des enjeux. Sur toutes ces questions, l'école française de psychomotricité (depuis l'œuvre fondatrice d'Ajuriaguerra) témoigne depuis cinquante ans d'une fécondité tant théorique que clinique centrée sur les liens complexes entre corps et psyché, au cœur des préoccupations psychopathologiques, mais plus encore thérapeutiques, des équipes pluridisciplinaires de la pédopsychiatrie. Véritable chaînon manquant (trop délaissé à nos yeux dans les réflexions psychodynamiques francophones et quasi ignoré dans les perspectives cognitivodéveloppementales plus « fondamentalistes » de nos collègues anglo-saxons), la sphère psychomotrice et les pratiques soignantes de la psychomotricité semblent pourtant un espace intermédiaire d'une extraordinaire pertinence pour nos élaborations psychopathologiques et surtout — versus thérapeutique — pour nos jeunes patients. Un double regard : 1) historicoépistémologique (sur l'identité de ces praxis psychomotrices) ; et 2) technique (cadre, processus, position du thérapeute, place des médiations et techniques, indications) nous conduit à réévaluer aujourd'hui le sens de ces thérapeutiques psychomotrices, c'est-à-dire aussi bien la direction historique et sociale de ce champ de compétences et des professionnels qui conduisent ce type de thérapeutiques à médiations corporelles, que la signification profonde de ces praxis, leur spécificité, leur efficace et leurs indications différentielles.

From the baby to the teenager, from the affect to the representation, from the most instrumental symptom to the most invading syndrome, from autistic paradigm to hyperkinetic syndrome, through numerous psychopathological problems, large-scale theoretical questions, (dynamic and developmental) psychological princeps issues, and how many essential technical and therapeutic issues intersect with body experiences which psychomotor sphere encircles a big part of the stakes. About all these questions, the French school of psychomotility (since the founder work of Julian de Ajuriaguerra) testifies since fifty years for a fertility so theoretical than clinical concentrated on the complex link between body and psyche in the psychopathologic even more therapeutic concerns of the multidisciplinary teams of the child psychiatry. Real missing link (too much abandoned for us in the reflexions French speaking psychodynamics and almost ignored in the perspectives more "fundamentalist" cognitive and developmental of our English colleagues), the psychomotor sphere and the medical practices of the psychomotility seem nevertheless an intermediate space of an extraordinary relevance for our debates, for our elaborations and especially — versus therapeutics — for our young patients. A double glance: 1) historic and epistemologic (on the idendity of these psychomotor praxis), and 2) technique (frame, process, position of the therapist, the place of the mediations and the techniques, the differential indications) leads us to revalue the "sense" of the today therapeutic psychomotor, that is as well the historic and social direction of this field of skills and the professionals who lead this type of therapeutic to physical mediations, that the underlying signification of these praxis, their specificity, their effective and their "differential" indications.
