Periprocedural Management of Oral Anticoagulation in Patients With Atrial Fibrillation: Approach in the Era of New Oral Anticoagulants

A growing number of patients with atrial fibrillation are treated with oral anticoagulation (OAC), and a large proportion of them will require surgical or other invasive procedures. These procedures typically involve interruption of OAC with or without the use of heparin bridging; however; there has been a dramatic change in this practice during the past 3 years. The introduction of short-acting, new oral anticoagulants and the growing popularity of some low-risk procedures with continued OAC have transformed our practice. Physicians and surgeons who treat patients with atrial fibrillation must be aware of these changes and understand how to deal with the new challenges they may encounter.

RésuméUn nombre croissant de patients ayant une fibrillation auriculaire sont traités par une anticoagulation orale (ACO), et une large proportion de ceux-ci auront besoin d’une intervention chirurgicale ou d’une autre intervention effractive. Ces interventions impliquent spécifiquement l’interruption de l’ACO avec ou sans substitution par l’héparine. Cependant, un changement notable dans cette pratique est apparu au cours des 3 dernières années. L’introduction de nouveaux anticoagulants oraux d’action rapide et la popularité croissante de certaines interventions ayant un risque faible avec l’ACO continue ont transformé notre pratique. Les médecins et les chirurgiens qui traitent les patients ayant une fibrillation auriculaire doivent connaître ces changements et comprendre la manière de faire face aux nouveaux défis qu’ils peuvent rencontrer.
