Indices d'intervention de l'équipe mobile de soins palliatifs : un outil d'autoévaluation et de valorisation de l'activité palliative hospitalière

A tool, consisting of eight indicators, was designed to explain and promote the palliative activity in a hospital, including the partnership between the palliative mobile care team and services identified with beds for palliative care. At the end of each stay, an index is attributed by the palliative mobile care team that reflects the resources and skills mobilized for the patient during his stay, and that reflects the collaboration between the palliative team and care service. The list of indices is transmitted monthly to the hospital information system team. If the information given by the palliative mobile care team index allows it, the encoding performed by the referent service is then optimized by the hospital information system service. The assignment of the indices is easy to learn, the procedure is simple, reliable, and reproducible because it eliminates any subjectivity. This tool allows a considerable time saving both for the palliative mobile care team and medical information department, while strengthening the finesse and quality of information transmitted. The implementation of the tool has other benefits: self-assessment for the palliative mobile care team, institutional recognition, and in the end proof of the need for palliative mobile care teams in the hospital group and its role as the cornerstone.
