Case ReportPrimary Cardiac Angiosarcoma Treated With Positron Emission Tomography/Magnetic Resonance Imaging-Guided Adaptive Radiotherapy

Radiotherapy (RT) for inoperable patients with primary cardiac sarcomas or residual tumor is often limited by the sensitivity of the heart and lung to radiation injury. We describe a novel treatment modality with adaptive radiotherapy (ART) using tumor volume tracking in a 37-year-old woman who presented with unresectable primary cardiac angiosarcoma. The patient was treated using positron emission tomography/magnetic resonance imaging-guided ART with 55.8 Gy concomitant with paclitaxel chemotherapy. In conclusion, the treatment was well tolerated, and a significant tumor volume reduction of ∼ 57% was achieved during radiotherapy, suggesting the effectiveness and tolerability of ART in combination with paclitaxel-based chemotherapy.

RésuméLa radiothérapie des patients inopérables qui ont des sarcomes cardiaques primitifs ou une tumeur résiduelle est souvent limitée par la sensibilité du cœur et des poumons aux lésions induites par le rayonnement. Nous décrivons une nouvelle modalité thérapeutique par la radiothérapie adaptative (RTA) à l'aide du suivi du volume de la tumeur d'une femme de 37 ans qui a présenté un angiosarcome cardiaque primitif non résécable. La patiente a été traitée au moyen de la tomographie par émission de positons et la RTA guidée à l'aide de l'imagerie par résonance magnétique utilisant 55,8 Gy concomitamment à la chimiothérapie par paclitaxel. En conclusion, le traitement a été bien toléré et a montré une réduction significative du volume de la tumeur de ∼57 %, qui a été atteinte durant la radiothérapie, ce qui suggère l'efficacité et la tolérabilité de la RTA en combinaison avec la chimiothérapie par paclitaxel.
