Quelques réflexions sur l’incidence des valeurs éthiques sur le choix technique des interventions psychothérapeutiques

RésuméSi l’on considère l’évolution de la clinique au cours du siècle dernier, on découvre l’évolution des valeurs éthiques implicites dans la théorie et ses implications sur nos choix techniques. La relation thérapeutique est asymétrique non seulement à cause des rôles dévolus au patient et au clinicien mais surtout à cause du pouvoir réel et fantasmé attribué à ce dernier. Le thérapeute est reconnu, d’une part consciemment, en situation de supériorité du fait de sa capacité professionnelle, et d’autre part inconsciemment, de par les phénomènes de transfert et la projection des fantasmes du patient. Parallèlement à la conduite du processus thérapeutique, le professionnel est aussi chargé de gérer l’asymétrie de pouvoir dans la relation. Ses choix théoriques et techniques reflètent l’usage qu’il fait de ce pouvoir, et la manière dont il s’en sert pour établir l’alliance thérapeutique.

If we consider the evolution in the patterns of psychopathology during the 20th century, we discover the evolution of ethical values implicit in the theory and their implications as far as our technical choices are concerned. The therapeutic relationship is asymmetrical, not only because of different role positions but also because of an inherent power differential. The therapist is consciously recognized as occupying a superior role because of his or her professional status and at the unconscious level, too, through transfer and projection of different fantasies. Being in charge of the therapeutic process carries with it the professional responsibility for handling this asymmetry. The therapist's theoretical and technical decisions in this regard reflect his or her ideology about power and how it should be used to establish the therapeutic alliance.
