Trouble des conduites : compréhension et typologie proposée

RésuméDepuis la première édition du Diagnostic and Statistical Manual of Mental Disorders (DSM), l’étude des comportements antisociaux et agressifs chez les jeunes a mené à une évolution considérable de la conceptualisation du diagnostic du trouble des conduites (TDC) [56], [57] and [58]. Ainsi, les recherches cliniques démontrent que de nombreux délinquants présentent des pathologies psychiatriques en co-occurrence à leur TDC, telle l’association entre le TDC et le trouble déficitaire de l’attention avec hyperactivité (TDAH). Au cours de la dernière décennie, une présentation atypique de la maladie affective bipolaire (MAB), affectant principalement la clientèle pédiatrique, a retenu l’intérêt des cliniciens. L’association entre le TDC et cette forme atypique de MAB a fréquemment été rencontrée auprès de jeunes délinquants particulièrement agressifs, irritables et impulsifs. Cette co-occurrence, identifiée comme étant le trouble dysphorique des conduites, représente une typologie nouvelle dans la conceptualisation des troubles du comportement. La validation d’une forme affective du TDC constitue un enjeu clinique important qui permettrait le développement de nouvelles approches thérapeutiques adaptées à un type de TDC traitable en psychiatrie.

Since the first edition, the evolution of the Diagnostic and Statistical Manual of Mental Disorders (DSM), studies related to juvenile antisocial and aggressive behaviors have led to an important evolutionary conceptualization of the conduct disorder (CD) diagnosis. Clinical research shows the majority of delinquents suffer from other psychiatric disorders in association to their CD, such as the well-documented comorbidity of CD and attention-deficit/hyperactivity disorder (ADHD). However, during the last decade an atypical presentation of bipolar disorder (BPD), which primarily affects children and adolescents, has captured the attention and held the interest of clinicians. Frequently, a comorbidity of CD and this atypical BPD has been noted among aggressive, irritable and impulsive juvenile delinquents. This comorbidity, identified as dysphoric conduct disorder, represents a new typology in the conceptualization of conduct and behavior problems. The validation of an affective form of CD is an important clinical issue that would allow the development of new therapeutic approaches adapted to a type of CD that can be treated in psychiatry.
