Kératomycose grave à Fusarium solani induite par un corps étranger tellurique : à propos d’un cas au Sahara marocain

RésuméNous rapportons le cas d’une kératite grave à Fusarium solani survenue chez un jeune homme au Sahara du Maroc où sévit un climat aride. Ce patient rapporte, depuis une semaine, la sensation d’un grain de sable dans son œil droit suite à une tempête de sable. À l’admission, il présente un abcès cornéen. Malgré le diagnostic rapide et la mise en route d’un traitement à base d’antifongiques disponibles : amphotéricine B et natamycine collyres, l’affection continue à s’aggraver conduisant à l’énucléation de son œil droit. Devant une suspicion d’infection oculaire suite à un microtraumatisme par grains de sable véhiculés par les tempêtes, il est important de réaliser des prélèvements biologiques à la recherche, parmi d’autres, d’éléments fongiques. Il est aussi important de s’approvisionner en nouveaux médicaments antifongiques triazolés afin de traiter rapidement et efficacement d’éventuelles mycoses oculaires.
