La fratrie de l’adolescente anorexique : apports pratiques d’une étude clinique

RésuméLa prise en charge hospitalière des patientes anorexiques au CHU de Rouen s’organise dans une unité spécialisée, au sein de laquelle travaillent pédiatres et pédopsychiatres. Un des outils thérapeutiques de cette unité consiste en l’introduction d’un contrat de soin avec notamment une phase de séparation entre la patiente et sa famille. L’accompagnement des parents durant cette hospitalisation avec des entretiens et leur participation à un groupe de parole est une pratique bien établie dans notre unité et décrite dans la littérature. Cependant l’accompagnement des autres membres de la famille et plus particulièrement de la fratrie semble le « parent » pauvre de la prise en charge autour de la patiente anorexique. Après un rappel non exhaustif de la littérature médicale autour de la prise en charge des fratries de patients souffrant de maladies chroniques, nous reprendrons les données concernant les fratries de patientes anorexiques. Dans une deuxième partie nous présenterons la méthodologie et les résultats d’une étude multicentrique prospective qui a associé durant deux ans une équipe de pédopsychiatres et de psychologues. Cette étude met en évidence, de façon objective, à travers des échelles psychométriques la souffrance psychique des membres de ces fratries particulières. Dans notre discussion nous aborderons les implications que nous pouvons tirer de cette recherche pour la pratique clinique afin d’aider ces frères et sœurs au mal-être parfois éclipsé par la lourde pathologie de leur germain hospitalisé.

The clinical management of anorexic patients is done in a specialized unit at the University Hospital Center of Rouen, within which work pediatricians and child psychiatrists. One of the therapeutic tools of this unit consists of the introduction of a “contract of care” with a phase of separation between the patient and his/her family. The accompaniment of the parents during this hospitalization, comprising of discussions and their participation in a discussion group, is a practice well established in our unit and described in literature. However, the accompaniment of other members of the family and more especially the siblings seems to be the neglected part in the clinical management of the anorexic patient. After a non exhaustive recall of medical literature regarding the siblings of patients affected with chronic diseases, we shall reconsider the findings of the siblings of the anorexic patients. Secondly we will discuss the methodology and the results of a multicentric long-term study which linked during this 2 years a team of child psychiatrists and psychologists. This study emphasizes, in a objective way, through psychometric scales, the suffering of the members of these particular groups. In our discussion, we shall bring up the implications which we can use for clinical practice to help these brothers and sisters in their sufferings sometimes concealed by the heavy pathology of their hospitalized sibling.
