آشنایی با موضوع

بروز یا در همه‌گیرشناسی(Incidence)، به احتمال پیش آمدن یک بیماری، در یک جمعیت، در یک دورهٔ مشخص از زمان گفته می‌شود. اگرچه گاهی برای بیان ساده‌تر، از آن به عنوان تعداد موارد تازه‌رخ‌دادهٔ یک بیماری در طول یک دورهٔ زمانی یاد می‌شود، ولی بهتر است آن را با یک نسبت یا شدت که دارای یک مخرج است، نشان داد. نسبت بروز یا بروز تجمعی یا بروز انباشه به تعداد موارد جدید در یک دوره زمانی مشخص تقسیم بر اندازهٔ جمعیتی که از آغاز در معرض خطر بوده است، گفته می‌شود. برای نمونه، اگر یک جمعیت در آغاز دارای ۱۰۰۰ فرد غیربیمار باشد و در طول ۲ سال مشاهده، در ۲۸ نفر از آنها شرایط یک بیماری گسترش پیدا کرده باشد، نسبت بروز برابر است با ۲۸ مورد در هر ۱۰۰۰ نفر در هر ۲ سال، یعنی ۲٫۸ درصد. میزان بروز عبارت است از: تعداد موارد جدید بیمارى که در یک جمعیت معین در یک دوره زمانى مشخص بروز مى‌کند. فرمول بدست آوردن آن چنین است: ( m/n ) × ۱۰۰۰ = I جمعیت در معرض خطر در همان دوره و در همان منطقه = n تعداد موارد جدید بیمارى در یک دوره معین در یک منطقه مشخص = m از تعریف بالا معلوم مى‌شود که میزان بروز اطلاق مى‌شود به: - فقط موارد جدید - در یک دوره معین - در یک جمعیت معین یا جمعیت در معرض خطر، مگر مخرج کسر دیگرى تعیین شده باشد - همچنین مى‌توان آن را به موارد جدید دفعات یا حملات بیمارى (Episodes) در یک دوره زمانى معین - در هزار نفر - اطلاق کرد. ۱۰۰۰ × (h/f) = میزان بروز (نوبت‌هاى بیماری) میانگین تعداد افرادى که در آن دوره در معرض خطر بوده‌اند = f تعداد دفعات بیمارى که در یک دوره زمانى معین آغاز شده = h میزان بروز، دفعات بروز جدید بیمارى را در جامعه اندازه‌گیرى مى‌کند و تحت تأثیر مدت بیمارى نیست و کاربرد آن عموماً به بیمارى‌هاى حاد محدود مى‌شود. میزان حمله بیمارى (Attact Rate) میزان حمله یک میزان بروز است (که به‌طور معمول برحسب درصد بیان مى‌شود) و فقط هنگامى بکار گرفته مى‌شود که جمعیت در یک دوره محدود زمانى در معرض خطر قرار گرفته باشد. (مانند هنگام بروز همه‌گیرى‌ها). این میزان تعداد موارد بیمارى را در جمعیت در معرض خطر با هم مرتبط مى‌کند و بازتاب دامنه همه‌گیرى است. فرمول محاسبه میزان حمله چنین است: (t/v) × ۱۰۰ = A. R مجموع جمعیت در معرض خطر در همان فاصله زمانى = t تعداد موارد جدید یک بیمارى معین در یک فاصله زمانى مشخص = v میزان حمله ثانویه (Secondary Attact Rate-SAR) عبارت است از تعداد اشخاص در معرض خطر که در فاصله ى دامنه دور کمون پس از تماس با یک مورد اولیه بیمارى در آنها بروز مى کند. موارد کاربرد میزان بروز میزان بروز بعنوان یکى از نشانگرهاى وضعیت بهداشتى در موارد ذیل بکار مى‌رود: - اقدامات مبارزه با بیمارى‌ها - پژوهش سبب‌شناختى و بیمارى‌زایى و توزیع بیمارى و کارآیى تدابیر پیشگیرى و اقدامات درمانی به‌عنوان مثال: هرگاه میزان بروز بیمارى رو به افزایش باشد ممکن است نشانه نارسایى یا کارآمد نبودن برنامه‌هاى جارى مبارزه با بیمارى‌ها باشد. افزایش موارد بروز ممکن است نشانه نیاز به فراهم کردن برنامه‌هاى جدید براى مبارزه با بیمارى‌ها و پیشگیرى از آنها باشد، یا لازم شود نظام گزارش‌دهى بیمارى بهبود یابد، تغییر یا نوسان در بروز یک بیمارى ممکن است به‌معناى تغییر در عامل سببى بیماری، یعنى تغییر در ویژگى‌هاى عوامل بیمارى‌زایی، میزبان یا محیط‌ زیست باشد.
