آشنایی با موضوع

اکستروفی مثانه ناهنجاری است که در آن مثانه از قسمت تحتانی شکم کاملا بیرون است و مجرای ادرار نیز کاملا باز است. این ناهنجاری طیف وسیعی دارد. فرم خفیف آن به صورت باز بودن مجرای ادرار در قسمت انتهایی در سصح پشت آلت می شود که به آن اپیسپادیازیس گویند. در فرمهای شدیدتر طول بیشتری از مجرا باز است به شکلی که گردن مثانه وخود مثانه را درگیر میکند. که در این صورت استخوان لگن هم از قسمت جلو باز می باشد. این ناهنجاری اغلب در پسرها دیده می شود و اغلب با ناهنجاری های سیستم تولید مثل و مقعد همراه می باشد. اکستروفی مثانه یک بیماری ژنتیکی و نادر است. در این بیماری به دلیل کامل نشدن جدار شکم در دوره جنینی، مثانه خارج از بدن قرار دارد و شامل طیف وسیعی از ناهنجاری ها در دستگاه ادراری تناسلی و استخوان لگن می باشد. احتمال بروز این بیماری در نوزادان پایین و در حدود یک تولد در 30000 تولد است. که این تعداد در نوع کلواکال آن به یک تولد در هر 100000 تولد نیز می رسد. درمان این ناهنجاری جزء مشکل ترین درمان ها در جراحی اطفال است. درمان اولیه باید در ۴۸ ساعت اول بدو تولد صورت گیرد. اعمال جراحی متعدد برای ترمیم مجاری ادرار و آلت تناسلی و برطرف کردن مشکل بی اختیاری ادرار لازم است که با توجه به نظر جراح مجرب در زمان مناسب صورت می گیرد. حوصله و تجربه جراح و همکاری والدین در موفق بودن درمان این بیماری اهمیت بسزایی دارد. باید اذعان کرد که عمل جراحی اکستروفی مثانه باید در مراکز آشنا به مسایل آن صورت گیرد. به طور کلی میزان موفقیت در درمان(که به معنای کنترل ادرار به مدت ۳ساعت می باشد)حدود ۵۰%است که وابسته به نوع ناهنجاری و عوامل ذکر شده در بالا دارد.
