آشنایی با موضوع

پروپرانولول (به انگلیسی: propranolol) (بانام‌های تجاری ایندرال و پرانول)، دارویی از دسته بتابلاکرهای غیرانتخابی است که عمدتاً برای درمان فشارخون و نامنظمی تپش قلب مورد استفاده قرار می‌گیرند. پروپرانول برای بیماری‌های گوناگون دیگری چون تسکین درد قلبی، پیشگیری از سردردهای میگرنی، درمان بی‌نظمی تپش و گلوکوم تجویز می‌شود. فرمول شیمیایی پروپرانول C۱۶H۲۱NO۲ است. پس از ۱ تا ۳ ساعت به حداکثر غلظت پلاسمایی می‌رسد و نیمه عمر آن ۴ تا ۵ ساعت است. اما زمان اثرگذاری آن طولانی‌تر از نیمه عمرش است و ممکن است تا ۱۲ ساعت طول بکشد. بیش از ۹۹٪ آن از راه مدفوع از بدن خارج می‌شود. پروپرانولول بلوک کننده غیراختصاصی گیرنده‌های بتا یک و بتا دوی سمپاتیک می‌باشد. نحوه‌ مصرف‌ مقادیر متفاوتی‌ از پروپرانولول‌ برای‌ علل‌ مختلف‌ تجویز می‌شود. شکل‌ طولانی‌ اثر پروپرانولول‌ معمولاً روزی‌ یک‌ بار مصرف‌ می‌شود. مهم‌ است‌ که‌ دارو را هر روز ساعت‌ معینی‌ بخورید. معمولاً بهتر است‌ دارو را صبح‌ پیش‌ از صبحانه‌ مصرف‌ کنید تا فراموش‌ نکنید. پروپرانولول‌ را می‌توان‌ با یا بدون‌ غذا مصرف‌ کرد. هر روز پیش‌ از خوردن‌ پروپرانولول‌ نبضتان‌ را بشمارید و فشارتان‌ را اندازه‌ بگیرید اگر کمتر از 60 در دقیقه‌ بود، یا فشار سیستولی‌ (عدد بالایی‌) کمتر از 90 میلیمتر جیوه‌ بود، پیش‌ از خوردن‌ داروی‌ آن‌ روز با پزشکتان‌ مشورت‌ کنید. هیچگاه‌ بیشتر از مقدار تجویز شده‌ مصرف‌ نکنید. از دستورات‌ پزشکتان‌ به‌ دقت‌ پیروی‌ کنید. اگر یک‌ نوبت‌ را فراموش‌ کردید، به‌ مجردی‌ که‌ به‌ یاد آوردید مصرفش‌ کنید. البته‌ اگر کمتر از 8 ساعت‌ تا نوبت‌ بعدی‌ مصرف‌ دارو رسیده‌ است‌، نوبت‌ فراموش‌ شده‌ را رها کرده‌، به‌ برنامه‌ منظم‌ دارویی‌تان‌ بازگردید. اگر بیش‌ از یک‌ نوبت‌ در روز پروپرانولول‌ مصرف‌ می‌کنید، اگر تقریباً موقع‌ نوبت‌ بعدی‌ مصرف‌ دارو رسیده‌ است‌، نوبت‌ فراموش‌ شده‌ را مصرف‌ نکنید. مقدار دارو را دو برابر نکنید و دارو را بدون‌ هماهنگی‌ با پزشکتان‌ قطع‌ نکنید. با قطع‌ مصرف‌ پروپرانولول‌ ممکن‌ است‌ در معرض‌ خطر یک‌ حمله‌ قلبی‌ باشید. در اکثر مواردی‌ که‌ پروپرانولول‌ تجویز می‌شود، بیماری‌ را مهار می‌کند و نه‌ درمان‌. و این‌ بدین‌ معناست‌ که‌ ممکن‌ است‌ لازم‌ باشد چندین‌ سال‌ یا تا پایان‌ زندگی‌ آن‌ را مصرف‌ کنید. هشدارها و عوارض‌ جانبی‌ در صورت‌ بروز هر یک‌ از علایم‌ نادر ولی‌ جدی‌ زیر، مصرف‌ آتنولول‌ را قطع‌ کرده‌، با پزشکتان‌ تماس‌ بگیرید: اشکال‌ در تنفس‌ تنگی‌ نفس، یا خس‌خس‌ سینه‌، سرد شدن‌ کف‌ دست‌ و پا، ضربان‌ قلب‌ زیر60 در دقیقه‌، سرگیجه‌ یا سیاهی‌ رفتن‌ چشم‌ در هنگام‌ بلند شدن‌ از حالت‌ خوابیده‌ با نشسته‌ با تورم‌ دستها و پاها یا افزایش‌ وزن‌ ناگهانی‌ (1/5 کیلو در 48-28 ساعت‌)، افسردگی، گیجی، یا توهم‌ (شنیدن‌، دیدن‌، یا احساس‌ چیزهایی‌ که‌ وجود ندارند)، درد قفسه‌ سینه، مفاصل‌ یا کمر، تب، گلودرد، بثورات‌ جلدی‌، یا هرگونه‌ کبودی‌ که‌ یا خونریزی‌ غیرعادی‌. نشانه‌های‌ زیر تا هنگام‌ عادت‌ کردن‌ بدنتان‌ با دارو ممکن‌ است‌ مشاهده‌ شوند: خواب‌ آلودگی‌، خستگی‌، مشکل‌ در خوابیدن‌، اضطراب‌، یبوست‌ یا اسهال، کابوس‌، یا کاهش‌ توانایی‌ جنسی‌، اگر این‌ علایم‌ ادامه‌ یافتند یا مشکل‌ ساز شدند، وضعیت‌ را با پزشکتان‌ در میان‌ بگذارید.
